ADHERENCE INTELLIGENCE IN PRACTICE
Measuring Revenue and Contribution From Completed Care
Separate observed collections, modeled scenarios and incremental results.
Activity, revenue and contribution are different
A message sent, an appointment booked, a visit completed, an invoice raised and money collected are separate events. “Recovered revenue” becomes ambiguous when those events are combined. Choose a definition before launch and show the event-level evidence behind the number. If payment is still outstanding, report it as outstanding.
Collected contribution also differs from gross revenue. It accounts for agreed relevant care-delivery costs. An evaluation of net program value must then account for additional program, implementation and staff costs without double-counting them. Neither a patient’s modeled lifetime value nor the amount on an unpaid invoice is cash recovered by an intervention.
A worked example with explicit assumptions
Consider a hypothetical cohort with 40 completed, paid visits after outreach at $200 per visit: $8,000 in gross collections. That is an observed total, not an incremental result. If a credible comparison suggests 30 comparable visits would have occurred under usual follow-up, the estimated difference is 10 visits, or $2,000 in gross collections.
Assume, for illustration only, relevant variable delivery costs of $80 for each additional visit. The estimated contribution difference is then $1,200. If incremental program and staff costs are $900, the modeled net difference is $300. These invented inputs illustrate the calculation; they are not Synarmic results, clinic margins or a forecast. Uncertainty in the comparison carries through every number.
Hypothetical amounts from the worked example, not Synarmic outcomes. The comparison estimate is uncertain.
| Calculation | Illustrative amount |
|---|---|
| 10 additional paid visits × $200 | $2,000 gross difference |
| Less 10 × $80 delivery cost | −$800 |
| Contribution difference | $1,200 |
| Less program and staff costs | −$900 |
| Modeled net difference | $300 |
All inputs are invented to demonstrate the calculation.
Build a collection and cost ledger
For each eligible patient, link the agreed care event to its completion record and relevant collected payment, including refunds or reversals. Define how bundled memberships are handled so a single payment is not counted as new revenue for every visit. Agree a collection window and keep outstanding balances visible.
Record staff time for reviewing cases, answering replies and managing escalations, as well as setup work. Explain which costs are incremental and which are allocated for analysis. A fixed salary is not automatically a cash saving when a task takes less time; the team may instead gain capacity. Report the capacity change separately unless a financial saving is actually observed.
Report the result with its limits
Show the eligible population, follow-up period, comparison method, care outcomes, collections and cost assumptions together. Small cohorts, incomplete follow-up, seasonal demand and differences between patient groups can change the estimate. Assignment to groups alone does not establish that the program caused a difference.
Include appropriate pauses, opt-outs, complaints and escalation handling alongside commercial measures. A care program should not pursue payments at the expense of clinical judgment or patient preference. The useful question is whether appropriate care improved with acceptable patient experience and staff effort, and what contribution was collected after relevant costs.
Practical review checklist
- Define what is being measured. A completed visit, an invoice and a collected payment are different events. Agree the collection window and cost definitions. Use contribution when discussing value after direct care-delivery costs.
- Establish the counterfactual. Some care would have been completed without the intervention. A suitable comparison helps estimate the incremental difference. Report the method and limitations rather than attributing all participant revenue to the program.
- Keep operational costs visible. Include implementation and program costs when estimating net value. Staff effort includes responding to patient messages, reviewing exceptions and handling escalations. Appropriate treatment pauses should not be treated as lost revenue to be recovered at any cost.
